2025 Child Care Survey
We need your help! As we plan for 2026, we need to hear from you so that we can focus our efforts where they are most needed.

You will be able to enter to win a gift card below and by entering your contact information at the end of your survey. We will be giving away TWO $100 gift cards soon! The winners will be selected from the 1st 25 parents and the 1st 50 providers.
Our goal is to hear from as many providers and families in child care programs in our service area as possible. Carefully read the descriptions below to determine which of the four surveys best fit your role.
Family Child Care Owner – To be completed by Family Child Care Providers located in the Central East CCR&R service area. Estimated completion time: 20-40 minutes.
Center and Head Start Leaders – To be completed by only one person in a leadership role at any child care program (except for FCC). This survey will obtain both provider and program information. Estimated completion time: 30-60 minutes.
Provider Only – To be completed by anyone working in a child care program not selected to provide program information in the Leaders Survey. Estimated completion time: 20-30 minutes.
Parents & Guardians – To be completed by parents/guardians that live/work in our service area with children ages 0-12 enrolled in a child care program. Estimated completion time: 15-30 minutes.
You will have the opportunity to return to your survey if you cannot complete it in one sitting.
